Glenwood Behavioral Health Hospital is dedicated to developing relationships with other healthcare professionals in the community. By partnering with referral sources, we are able to widen access to clinically effective mental health and dual diagnosis programming.
Efficient Referral Process
A patient’s treatment shouldn’t be delayed because of a disorganized or inefficient referral process. Glenwood Behavioral Health Hospital offers an efficient referral process that is seamless for both provider and patient.
You are able to contact a member of our team 24 hours a day, seven days a week to refer a patient to Glenwood Behavioral Health Hospital.
During that first conversation with one of our team members, we will gather some basic information from you to determine whether our facility is the best fit for your patient’s needs.
If Glenwood Behavioral Health Hospital is the right fit for your patient, we will coordinate directly with them to finalize their intake assessment, verify their insurance coverage, and schedule their admission date.
Once we receive your patient’s permission, we will update you on their admission and discuss how we’ll share information about their progress with us.
Who We Partner With
At Glenwood Behavioral Health Hospital, we recognize how valuable relationships with other healthcare professionals are, and we collaborate with referring providers to get their patients timely, appropriate treatment.
We partner with most referring providers whose patients can benefit from our services, but the most common referral sources we work with include:
- Psychologists
- Counselors
- Physicians
- Community agencies
If you think that our services might be right for you or someone you care about, we also accept self-referrals or referrals from a loved one.
This does not represent a conclusive list of the healthcare professionals we partner with. We encourage providers who are interested in referring patients to us to reach out to us directly.
Benefits of Referring to Us
Collaboration and communication are foundational to our referral relationships, ensuring that each patient receives focused, patient-centered care.
Referring providers benefit from a smooth transition of care that keeps them involved every step of the way. With your patient’s permission, our team will share appropriate information with you, including:
- Physical and psychosocial status
- Summary of care provided
- Progress toward goals
- Reason for discharge or referral
- Community resources provided to patient
- Referrals provided to patient
Our commitment to you as the referring professional is to provide you with a seamless referral experience that gets your patient the treatment they need with minimal delay.
We place tremendous value on the relationships we build with referring providers, and we look forward to working with other healthcare professionals in the community.
